VACATION LEAVE

Full-time
- 6 month waiting period
- Accrue up to 12 days per year
- Credited at the rate of 3.69 hours per pay beginning on first day of employment
Part-time
- Credited at 1/2 of the full-time annual leave benefit
SICK LEAVE
Full-time
- 90 day waiting period
- Accrue up to ten (10) days per year
- Credited at the rate of 3.08 hours per pay beginning on first day of employment
Part-time
- Credited at 1/2 of the full-time vacation leave benefit

RETIRMENT

SHORT TERM DISABILITY
- 90-day eligibility waiting period
- FREE to eligible employees
- Available to full-time and part-time employees who work at least 16 hours per week
- Benefit paid at 60% of base weekly salary
- Benefit effective on the 15th day following accident or illness up to 11 weeks
LONG-TERM DISABILITY
- 90-day eligibility waiting period
- FREE to eligible employees
- Available to full-time and part-time employees who work at least 16 hours per week
- Benefit paid at 66 2/3 % of base weekly salary
- Benefit effective following short-term disability
FAMILY AND MEDICAL LEAVE
- Available to employees who have worked 12 months or 1,250 hours
- 12 workweeks of unpaid leave during any 12-month period for the following reasons:
1.
For the birth and care of the newborn child
of the employee;
2.
Employee is unable to work because of a serious
health condition;
3.
Placement with the employee of a son or daughter
for adoption or foster care; or
4.
To care for an immediate family member (spouse,
child, or parent) with a serious health condition
LIFE/AD&D INSURANCE
Jefferson Pilot
- FREE to eligible employees
- Available to full and part-time employees who are scheduled to work at least 16 hours per week
- 30-day eligibility waiting period
- Benefit is 100% of annual salary to a maximum of $50,000
Fortis
- FREE to eligible employees
- Available to full and part-time employees who are scheduled to work at least 20 hours per week
- 90-day eligibility waiting period
- Benefit is $5,000 per eligible employee
- Additional life insurance can be purchased by employee for $5,000; $15,000; $25,000; $35,000;
and $45,000
*Cost for additional coverage is $0.36/$1,000
